The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers. To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city. It facilitates partnerships, often new and unconventional, between government and the private sector to develop, test, and launch new initiatives. These collaborations speed the execution of demonstration projects, effect expansion of successful pilot programs, and support rapid implementation to meet the public health needs of individuals, families, and communities across New York City.

PROGRAM OVERVIEW

This role sits in the Bureau of Brooklyn Neighborhood Health in the Center for Health Equity & Community Wellness (CHECW), which aims to eliminate racial inequities resulting in premature mortality, with a focus on chronic disease, by addressing the social and environmental factors that impact health. CHECW works to increase placed-based investments in priority neighborhoods with community programming and services based on epidemiology; influence and leverage the health system to promote whole-person care; intensify the agency’s approach to tackling big salt, sugar, and tobacco; and finding innovative ways to improve the built environment and address other social determinants of health. CHECW is comprised of the Bureau of Bronx Neighborhood Health, the Bureau of Brooklyn Neighborhood Health, the Bureau of Harlem Neighborhood Health, the Bureau of Chronic Disease Prevention, the Bureau of Health Equity Capacity Building, the Bureau of Equitable Health Systems and the Bureau of Finance, Administration, and Services. CHECW seeks to create healthier neighborhoods by providing New Yorkers information and resources needed to live healthy lives, and work to remove health inequities between New York City neighborhoods as a result of structural racism and disinvestment in social capital.

POSITION OVERVIEW

DOHMH is seekinga Community Health Workerto engage in multiple streams of Chronic Disease and COVID-19 prevention and recovery work until June 2024.

This role sits in the Bureau of Brooklyn Neighborhood Health in the Center for Health Equity & Community Wellness (CHECW), which seeks to eliminate racial and other inequities resulting in premature mortality. With an unwavering grounding in history and structural analysis, CHECW works to increase visibility of the harm perpetuated by centuries of racist, socially unjust policy while pushing towards redress for the most impacted NYC communities. CHECW addresses inequity across community and healthcare systems in partnership with community, faith-based, and health care organizations. CHECW’s work focuses on social determinants of health, including environmental and commercial determinants, and addresses both upstream and downstream factors to improve health and well-being of New Yorkers. CHECW is a division of the NYC Department of Health and Mental Hygiene.

As part of the NYC Public Health Corps, the Bureau of Brooklyn Neighborhood Health is seeking to hire a Community Health Worker to support efforts to address premature mortality ie: diabetes, hypertension, mental health, maternal health and COVID-19. This person willimprove the health status of neighborhoods by engaging the community, conducting outreach (e.g., neighborhood canvassing) and promoting resources, services, and programs for health and wellness. Under the supervision of the program director, the Community Health Worker will have the latitude to serve as a team lead in any phase of community programs, engagement, and outreach. This includes providing community engagement and programmatic support to address maternal mortality, mental health, diabetes, hypertension and COVID-19 recovery throughout Brooklyn.

RESPONSIBILITIES

  • Assist with planning and executing health awareness and promotion activities in diverse settings (some evening and weekends work required)
  • Engage community partners and local organizations, on health information and education resources to improve health to decrease rates of premature mortality.
  • Conduct outreach, develop relationships, and strengthen collaboration with community residents, organizations, and key stakeholders program partners to improve health equity in Brooklyn
  • Assist with program administration for community outreach and detailing work and provide clerical support as needed
  • Provide support in the emergency response efforts including but not limited to disseminating COVID-19 and Brooklyn Bureau programmatic information and resources (e.g., PPE) to increase access to and awareness of services
  • Assist with managing, monitoring, and tracking all programmatic resource distributed to the public
  • Support other programmatic needs within the Bureau
  • Other duties as assigned by supervisor

QUALIFICATIONS

  • High school graduation or equivalent and three years of experience in community work or community centered activities in an area related to duties described above
  • Quick learner and self-starter
  • Works well independently and within team
  • Some community engagement experience, a plus
  • Familiarity with data entry and tracking
  • Excellent verbal and written skills; presentation skills a plus
  • Experience working and/or engaging in priority neighborhoods
  • Strong interpersonal skills and the ability to build trust and maintain relationships with community partners
  • Strong organizational skills with good follow-through to complete assignments within specified timeframes

PREFERENCES

  • Bilingual in Spanish or French
  • Demonstrable experience in addressing drivers of premature mortality ie: diabetes, hypertension, mental health, maternal health and COVID-19
  • Ability to understand the concepts of social determinants of health, institutional racism, structural racism, health inequities and their impact on underserved and underrepresented communities
  • Demonstrated commitment to supporting communities who have experienced systemic oppression and bias (e.g., people of color, LGBTQ+ people, immigrants, justice involved persons, etc.)

HOURS

35 hours per week. This position will be funded through June 30, 2024, with possibility of extension.

COMPENSATION

FPHNYC offers a comprehensive benefits package. The salary for this position is $60,000 a year.

ADDITIONAL INFORMATION

There is potential for this position to transition to DOHMH and therefore candidates must meet DOHMH eligibility requirement including NYC residency. This position will operate in a hybrid work environment with the potential of shifting to 100% in-office.
