Population Health Management Medical Advisor

The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers. To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city. It facilitates partnerships, often new and unconventional, between government and the private sector to develop, test, and launch new initiatives. These collaborations speed the execution of demonstration projects, effect expansion of successful pilot programs, and support rapid implementation to meet the public health needs of individuals, families, and communities across New York City.

PROGRAM OVERVIEW (ELC Grant; Remote)

Help our city re-imagine a Post-COVID world by joining the Bureau of Equitable Health Systems (BEHS) in the Center for Health Equity and Community Wellness (CHECW) of the NYC Department of Health and Mental Hygiene (DOHMH).

About the Division – Center for Health Equity and Community Wellness

CHECW aims to eliminate racial inequities resulting in premature mortality, with a focus on chronic disease, by addressing the social and environmental factors that impact health. CHECW works to increase placed-based investments in priority neighborhoods with community programming and services based on epidemiology; influence and leverage the health system to promote whole-person care; intensify the agency's approach to tackling big salt, sugar, and tobacco; and finding innovative ways to improve the built environment and address other social determinants of health. CHECW is comprised of the Bureau of Brooklyn Neighborhood Health, the Bureau of Bronx Neighborhood Health, the Bureau of Harlem Neighborhood Health, the Bureau of Chronic Disease Prevention, the Bureau of Health Equity Capacity Building, the Bureau of Equitable Health Systems and the Bureau of Finance, Administration, and Services.

About the Bureau – Equitable Health Systems

The BEHS is the healthcare systems bureau of DOHMH. Our mission is to apply policy, evidence, and practical expertise to improve equity in health care delivery at the individual, organizational, and systems levels. We do this by engaging primary care providers and other healthcare organizations to implement evidence-based strategies; leveraging information to support planning and technical assistance for providers and payers; advancing policy to close the racial equity gap for priority health outcomes; and surfacing opportunities where health care can influence and connect consumers to social support and addressing the whole person, beyond physical ailments.

About the Grant purpose:

The COVID-19 emergency exposed the fragmentation of care in NYC. Payers and providers have access to data exclusively for their patients. DOHMH uses traditional public health surveillance tools (surveys or monitoring), which either don’t allow for the identification of payer/provider or only identify payor/provider for a very specific subset of the population. As a result, there is limited-to-no strategic public health guidance for the management of priority populations with clear individual-level mapping of payers and providers. These limitations are made all the more apparent during emergencies, hindering the health department’s ability to plan and provide targeted outreach to high-risk patients via payors and providers. Populations suffering inequities such as Black pregnant people, Black chronically ill people, or the homebound are prime subpopulations to prioritize during emergency and non-emergency periods to prevent further marginalization.

To address this issue, we will create a blueprint for DOHMH to utilize its individual-level data sources to catalyze population health management for all NYC residents. This workstream lays the groundwork for a way to analyze currently disparate data sources on New Yorkers’ health, insurance, and social information to inform city-wide population health management strategies that eliminate inequities.

POSITION OVERVIEW

DOHMH maintains a variety of individual-level data sources (e.g. diabetes registry, vital statistics, immunization registry, etc), and has a vehicle to access at individual-level data from additional data sources (RHIOs, Medicaid Data Warehouse, SPARCS, databases connected to the Health and Human Services Common Client Index which include information on homeless shelters, SNAP, among others). The theoretical level of data access that DOHMH has is one of the most comprehensive any health organization could have. However, this access could be better leveraged to catalyze collective action of the healthcare system to improve inequities in access, care, and outcomes.

The Population Health Management Medical Advisor will co-lead a 10-month task and finish project to create a Blueprint for citywide, equity-focused, population health management of 3-5 population subsets (e.g. pregnant, chronically ill, homebound, etc).

RESPONSIBILITIES

  • Produce a landscape analysis of the most comprehensive population health management efforts performed nationally or internationally; as well as trends in population management in the US.
  • Develop 3-5 use cases for collective, equity-focused, population health management in NYC.
  • Create a plan to directly reach patients in 3-5 population subsets with emergency communications.
  • Coordinate with the legal team to identify data sharing needs and opportunities to operationalize the Blueprint.
  • Provide medical and population health management guidance to the Bureau of Equitable Health Systems and collaborators.

QUALIFICATIONS

  • MD, DO, or NP
  • 3+ years of experience in population health management and/or analytics. Preference for experience in an insurance, ACO, or IPA setting.
  • 2+ years of experience in an inpatient hospital setting
  • Proficiency in guiding analyses of claims and other administrative data for population management purposes
  • Self-starter, creative mindset
  • Project management skills

SALARY

  • Salary range is $125,000 to $130,000.

RESIDENCY REQUIREMENT

You must live in New York City Tri-state area (NY, NJ, CT) in order to be considered for a position at FPHNYC.
