Dunwoody Village, a 5-star rated continuing care retirement community (CCRC) in Newtown Square of Delaware County, is seeking a dynamic Full-Time Social Worker within our Skilled Nursing and Short-Term Rehabilitation departments. This position does require working a weekend rotation along with our team of social workers.

The health and safety of our team members and residents are our most important priority. With this in mind, we do require that all Dunwoody Village employees be vaccinated against COVID-19 and Influenza. Exemptions based on religious beliefs and qualifying medical reasons will be considered.

This integral team member is responsible for completing biopsychosocial assessments and assuring comprehensive discharge plans for short-term residents. Additional duties of a Social Worker include but are not limited to:

  • Maintaining responsibility for completing required documentation, including mood and cognitive assessments, and for providing excellent customer service.
  • Leading care plan meetings and helping residents and their loved ones to understand their expected length of stay
  • Implementing and discuss care planned goals and interventions towards safe discharge planning
  • Acting as a liaison and point person for residents and responsible parties
  • Assisting with insurance case management and reports to the Director of Social Service.

To qualify for this position, candidates must meet the following criteria:

  • a Bachelor’s Degree in Social Work; candidates with a Master’s Degree are also encouraged to apply
  • minimum of 2 years of relevant experience, preferably in a similar setting and population
  • Working knowledge of Point Click Care and Microsoft Office – including Word and Outlook.
  • Ability and keen desire to work effectively in a team-based setting.
  • Willingness/desire to provide exemplary customer service to both internal and external customers
