CLINICAL SERVICES - PARTIAL CARE COUNSELOR/FT/UNION COUNTY

Partial Care Counselor

(Full-Time)

CenterPath Wellness, a division of Easterseals New Jersey, is seeking a Partial Care Counselor to assist with an array of clinical services for our clients and their families in our Plainfield New Jersey office!

Key responsibilities include, but not limited to:

• Reviews, updates, and utilizes the biopsychosocial to develop and implement treatment plans.
• Re-evaluates the client’s needs throughout treatment.
• Identifies/escalates issues where client symptoms are exacerbated, and action is needed to ensure client safety and well-being.
• Completes client discharge summaries at critical and appropriate timeframes.
• Provides group therapy on mental health topics.
• Documents all client interactions in the Electronic Health Record system within the required timeframes.

To Qualify, you will need:

• Bachelor's degree in social work, psychology, or related mental health field
• Some experience in the mental health field
• Preferably group counseling experience
• Computer proficiency with MS Office and Electronic Health Records
